We offer our New Patient Intake questionnaire online so you can complete it in the comfort of your own home. It saves you time in our office and it allows us to get started with your first appointment on time. If for any reason you aren’t able to complete it before your appointment time, we ask that you arrive 10-15 minutes prior to your appointment to complete the questionnaire in our office. Arriving at the time of your appointment without the intake questionnaire completed may result in needing to reschedule your appointment. Please follow these directions:
- Begin by selecting the most applicable option below. (Pediatric is for 12 years old and under, Teenage is for 13-18 years)
- Fill out each portion as thoroughly as possible.
- We will be notified once all forms are submitted.
